Patients must be 18 years of age or older.

Patients must be a resident in the state of Ohio and have a valid driver's license or state ID

The total cost of the doctor's evaluation and certification processing fee is $149.99.

There is an additional state registration fee to receive the state issued digital medical card. The additional state registration fee is $50.

Appointments take place online by webcam or telephone call at the appointment time.

If you are not approved, you will receive a full refund of the appointment fee.

The Ohio medical marijuana card is valid for one (1) year from the issue date.
