A doctor excuse is a legal form secured by a patient as proof of his condition. This is commonly used to excuse employees or students from taking the day off and employees will still get paid and students will be given makeup assignments or exams that he or she may have missed. Given the fact that this form is a legal form, it should give you an implication that you should not tamper with it, nor use a fake one.

An authentic excuse form must contain the following information: the doctor’s contact number, including his complete name and address, the date when the form was written or secured, the complete name of the patient and a certification of medical consultation, which contains the diagnosis, the reason for the absence, when the patient can go back to work and under which conditions can he start working again.

A doctor excuse can come in three different forms. One type is the letter head. This is the type that follows the template with all the information stated above. It is written on a letterhead stationary from the doctor’s office or clinic. Another type is the prescription pad. This is when a doctor uses his or her own prescription pad, with his complete name and information in the letterhead. The doctor then writes his prescriptions and medications. This is the best form to use when writing a short and simple note. The last type is the special form. This is also called the fill in the blank form. This is a pre-printed form from the hospital or the clinic that is filled up and signed by the attending physician.

For those who opt to use a fake doctor excuse, it can be very convenient as compared to securing one directly from the physician, especially if their reason for absence is not that critical. When employees just feel a little under the weather or has a simple cold or flu, they would prefer to just go online, pay for a fake excuse, customize it according to his or her requirements and print, as opposed to still passing by the doctor’s clinic.

A doctor excuse can be used for all sorts of illnesses. These include an excuse letter for any eye disease, dermatologist excuse for all skin related diseases, lung related illnesses such as asthma, bronchitis or the like, even mental illnesses or doctors.

In using a fake doctor excuse, it should be taken into consideration that if ever you get caught using one, there may be dire consequences. For students that are caught, they can be suspended, failed from the subject, or for college students, they can be expelled from their universities. For working people, they can be fired immediately from work if you are caught to have made up a doctor’s name for the note, and can even be involved in a legal case if you are suspected to have forged the doctor’s signature, which can cause you large amounts of fine or even serve time in jail.
